Grant Win Helps Orange County Health Centers Advance Community Care
With this HRSA Health Center Controlled Networks (HCCN) Program grant, the Coalition of Orange County Community Health Centers (the Coalition) will support 10 participating health centers, including federally qualified health centers (FQHCs) and FQHC look-alikes, serving over 125,000 medically underserved residents across 43 clinical sites.
Through tailored technical assistance and strategic collaboration, the coalition will enhance data management capacity, technology integration across systems, value-based care, and address social risk factors. Leveraging prior successes, the Coalition is well-positioned to continue advancing affordable, accessible, and high-quality health care throughout Orange County, California.
